What is membership medicine?
Membership (sometimes called concierge) medicine is a proactive style of medical practice that emphasizes wellness and disease prevention through highly personalized care.
Our new model is designed to foster a deeper partnership as your primary care physician, helping to ensure a strong, satisfying, and improved doctor-patient relationship. A membership fee is charged, allowing us to see fewer patients, so we may devote more time to each patient’s care and individual needs. Our smaller patient panel will allow us to provide ongoing education and support aimed at improving health outcomes.

How does our practice differ from the typical traditional practice?
My goal is to provide you and your family with improved health care and form a long-term partnership.
Patients will never feel rushed, so we can take the time to address your health concerns and questions. Because my practice panel size is limited, you will be able to contact me when you need advice, and you will be able to schedule an appointment quickly. I am accessible by text, phone, and patient portal anytime.
What does membership include?
Your membership includes benefits not covered by Medicare or insurance plans. These benefits provide improved management of health challenges. This is provided by ongoing education and support aimed at improving health outcomes, and routine exams to check on your health on an ongoing basis. We will focus on pre-condition health optimization, rather than reactive care. 24/7 communication support will be available to help you achieve your health goals!
What if I need urgent care?
If you have a life-threatening emergency, call 911 immediately. A family member or caregiver can call us later so we can be in contact with the emergency room.
For urgent problems that are not emergencies, you can contact us by phone, text or patient portal anytime to discuss your medical needs, and in most cases see us the next day!
Do you bill insurance plans?
What insurance plans do you accept?
We accept all major commercial medical insurance plans and medicare, including many medicare advantage plans.

Do I still need to have health insurance?
Yes, you do. Membership-based medical practices do not take the place of general health insurance coverage. However, by providing you with an improved healthcare experience, your plan utilization can often be reduced.
Can joining PNW Family Medicine actually save me money?
Good question. Many patients find that investing in their health provides a great return over the long run. I can properly handle the vast majority of your health issues and concerns, with the goal of identifying and preventing disease. We will provide ongoing education and support aimed at improving health outcomes.
How do I become a member?

Please remember that space is limited so we can devote more time and energy to each patient. We accept members on a first-come, first-served basis. Once we have reached our ideal practice size, we will establish a waiting list, so we encourage you to act quickly.
Can I terminate my membership?
Of course. You are free to cancel your membership at any time and for any reason. We require notice in writing, and upon receipt of your request, the cancelation takes effect after your current billing period. You will be allowed 30 days of emergency care after cancellation. Annual prepaid membership is not eligible for a refund. If you are dissatisfied with our practice in any way, please tell us, so we may improve our systems.
If I cancel my membership, can I reapply later?
You will need to contact us if you wish to restart membership at a later time. Please keep in mind that we are limiting the number of member patients in our practice. If our patient panel is full, you may be placed on a waiting list until space becomes available.
